Barnsley crashed out of the EFL Cup with a first round extra-time loss at home to League One Northampton Town on Tuesday.
Josh Scowen got the opener for the Reds but a bad error by goalkeeper Adam Davies gave the visitors the equaliser before the winner came after 114 minutes.
Paul Heckingbottom named a very strong side, despite making three changes from the team that lost 4-2 at Ipswich on the opening day of the Championship season. Marley Watkins, who came on after four minutes on Saturday for the injured Sam Winnall, started the cup tie while James Bree and George Moncur replaced Andy Yiadom and Ryan Kent. Moncur played just behind Tom Bradshaw in a 4-4-1-1 formation with Watkins on the right wing.
After a quiet opening 20 minutes – apart from a Conor Hourihane long-range effort which whistled just wide – Barnsley took the lead through Scowen. The central midfielder met Adam Hammill's excellent left wing free-kick seven yards out and headed into the bottom left corner.
The Reds dominated the half but did not really come close to doubling their lead – Alfie Mawson's wayward free-kick being the closest effort. Northampton provided very little threat until seconds before the break when Marc Roberts lost the ball and his central defensive partner Mawson had to deflect a Harry Beautyman shot wide.
Northampton were gifted an equaliser seven minutes into the second half when Davies punched the ball into his own net after on the line after Gabrial Zakuani headed a Matty Taylor corner. Davies then made a decent save from Kenji Gorre, son of ex-Red Dean.
Heckingbottom took off Moncur after an hour, bringing on Kent and moving Watkins up front. Kent instantly set up a chance for Watkins which was hit from the edge of the box straight at visiting goalkeeper Adam Smith who left dived to his left to keep out Hammill's long-ranger. Mawson blasted wide at the back post late on after meeting a Hourihane corner a couple of yards out at a difficult angle.
Two minutes into extra-time, Barnsley substitute Stefan Payne smashed a ferocious 25-yard shot against the top of the right post. Another substitute Ryan Kent also came close but goalkeeper Smith rushed off his line to deny him after a fine Watkins pass.
Watkins then blazed a great chance over the bar in the second half of extra-time before failing to beat Smith with a header.
But it was the visitors who netted when John Joe O'Toole headed in Taylor's cross midway through the second half of extra-time.Hammill's free-kick then hit the bar at the other end after Roberts appeared to get a touch but they could not find the leveller.
